Summary
Asian Services In Action’s International Community Health Center
(ASIA-ICHC) is a federally qualified health center with two sites,
based in Cleveland and Akron, Ohio. ASIA-ICHC predominantly serves
low-income, underserved, limited English-speaking Asian American and
Pacific Islander (AAPI) and other immigrant and refugee populations
of all ages.
Asia-ICHC seeks a motivated individual with connection to their
community members to provide culturally and linguistically
appropriate outreach and education to their respective community. He
or she must be outgoing, bilingual, available during non-traditional
work hours, be proficient with computer usage, adhere to Asia-ICHC’s
confidentiality policy, have a good follow-through on assigned
projects, and be committed to Asia-ICHC’s mission. The CHW will work
directly with ICHC Asst. Director of Operations and any other
assigned staff.
Duties and Responsibilities
1. Utilize informal networks (faith, business, social media and
ethnic social) to outreach and educate ethnic community on specific
health issues which include: TB, Type 2 diabetes, High Blood
pressure, immunization, childhood lead prevention, breast and
cervical cancer, Hepatitis B, HIV and other areas.
2. Conduct program recruitment and outreach to their respective
ethnic community.
3. Work with ICHC managers and staff to coordinate health screening
and presentation events in the community either virtually or
in-person; provide bilingual support at these events.
4. Assist CHP staff in language and cultural communication. This may
include activities such as:
a. Providing interpretation or support to CHP staff and others
during events.
b. Assist with translating educational documents, flyers, etc.
5. Assist in coordinating client services
a. Assisting people to seek follow-up medical services at the clinic
when necessary
c. Promoting and working with front desk staff to schedule
appointments at ASIA’s clinic with clients.
d. Informing clients about other community resources
e. Presenting health education in a culturally appropriate format in
person, virtual (e.g. Zoom) or social media (e.g. Facebook).
f. Providing social support to improve members’ adherence to medical
treatment and lessen the risk that they will drop out of program.
g. Helping clients with understanding program eligibility and
helping them complete enrollment forms, etc.
6. Participate in trainings and meetings as required for specific
grants and planned events.
7. Maintain outreach records and furnish collected information to
CHP staff in a timely manner. Maintain all client information in a
confidential manner in accordance with policy and HIPAA regulations.
Qualifications
1. Must reside in and share experience, language and/or culture of
the community to be served.
2. Must have valid driver s license and liability insurance
coverage.
3. Bi-lingual preferred
4. Minimum High School Diploma or GED required.
5. Minimum of 1-2 years of experience in health, social or community
services or comparable
6. Have access to Internet and computer at home.
7. Familiar with basic computer programs such as word, Excel, and be
able to connect virtually for meetings or education events (e.g.
Zoom ).
